Applications for the ACO Medical Director Scholarship for the December 2017 Caravan Health Accountable Care Symposium may be submitted beginning on Tuesday, August 1st.

 

Eligibility to attend the symposium at no cost is exclusive to ACO Medical Directors and contingent upon verification of ACO Medical Director status via the application process.

 

To apply, submit online now via the registration form above. Following submission, your application will be reviewed and you will be contacted at the email you provided with confirmation upon approval. You may expect the application approval process to take between 1-3 business days.

 

This scholarship valued at $900 will include the following:

 Travel Reimbursement

For all 2017 AIM/ACO participants, reimbursement criteria for travel for the Phoenix Symposium will be:
  • Travel accommodations must comply with the community’s internal travel policy
    • We offer special accommodation pricing at the hosting Hilton Squaw Peak Resort
 
  • Any travel reimbursement submissions will be deducted from that community’s quarterly maximum reimbursement.
    • Nearly all 2017 communities have excess funding and if they so choose, the funds can be used for this purpose.

For 2018 ACO participants, please refer to your internal communities travel policy. Caravan Health will not be reimbursing travel, we will, however, fully cover your Symposium registration and associated activities as noted in the scholarship above.  You or your health system will be responsible for individual travel and accommodation costs.  We very much look forward to welcoming you in Phoenix and we see participation at the Symposium as a great opportunity to get your ACO physician engagement off the ground quickly in the coming year!

Driving – Follow airport signs for 24th Street North (but do not exit 24th Street North). Go to State Route 51 North; follow 51 North to the Glendale Ave. exit. Exit Glendale Ave., turn left (west) onto Glendale Avenue to 16th Street, turn right (north). Follow 16th Street north to Morten Ave. (1st light) turn right (east) on Morten.
